Athletic Clearance

All players must have the following completed to participate as a member of the CCBV JV or Varsity Team. If players have participated in school or club sports and have completed some of the below, kindly note the dates after which items are accepted to ensure they are timely.

Checklist

To expedite registration at tryouts, please email Nos. 2 to 5 to admin@ccboysvolleyball.com. Scroll down the page for more information.

    3. Page 4 of sports physical (download PDF)
        • Accepted if dated after July 2023.
        • Form EL2 must be used.
    4. Signed waiver packet by parent(s) and athlete
        • To sign electronically, please email admin@ccboysvolleyball.com. The same email can be used for all as you’ll be prompted which role you’d like to e-sign. Please include in your email full name and emails for:
          • Parent/guardian 1
          • Parent/guardian 2
          • Athlete
    5. Player completion of free courses
        • Certificates accepted if dated after July 2023.
          • Concussion in Sports
          • Sudden Cardiac Arrest

Sports Physicals

We found the least expensive sports physicals offered by the Department of Health in Clay County, followed by Walmart Health. However, if anyone has a better option, please let us know so we can share! Below is the physical form and some choices for convenience.

Courses

After athletes complete a course, they’ll be able to download a copy of the certificate of completion. Both courses should take no more than an hour to complete. 

 

Directions

    1. Go to www.nfhslearn.com.
    2. Create a login and a password or sign in if you already have an account.
    3. Click on “Courses” in the top navigation bar.

  3. **You must complete the following for each course.***

     

    1. Search for the courses below or click on the links:
    2. Select Florida as your state.
    3. Click “Order Course.” (Videos are free.)
    4. Select an option for your athlete to take the course using your account, or for you to forward the course via email to your athlete.
    5. You’ll see the tutorial in your “Dashboard” by clicking that tab in the top navigation bar.
    6. The course can be stopped and completed in multiple sittings.
    7. For additional help viewing the course, please contact NFHS via the “Help” tab in the top navigation bar.
    8. After you complete each course, download a copy of the certificate.
